Massiva
A very, very nice freeware MIDI and audio sequencer by Jørgen Aase.
Maybe not as powerful as its big brothers Cakewalk, Cubase, Logic etc. but
  1. It's free
  2. It has a lot of non-standard ways to compose
  3. It supports VST
  4. It has a built-in softsynth(!)
  5. It has the most elegant interface of all sequencers I know
Hubi's MIDI loopback
Add up to four (virtual) MIDI In and MIDI Out ports to your setup. Any In port can be connected to any Out port. Freeware.
Maximum MIDI
MIDI Programmer's toolkit for C++ programmers.
KeyKit
The ultimate MIDI environment. A VERY flexible, programmable MIDI program. Offers lots of library functions and some basic editing tools; what is missing you can build yourself (programming knowledge needed).

A very nice algorithmic program from Lars Kindermann.
Quasi Fractal Composer
A MIDI program that uses fractal "self-similarity" functions to compose songs. IMO very "natural" sounding results.
Tangent
Tangent is a "top-down" composer that works with musical structures, not individual notes. It builds a composition by creating periods of bass and polyphony. It does this automatically but still allows user control. It can even take a simple melody and create an entire composition.
Softstep
SoftStep is a Windows 95/98/2000 based modular MIDI step sequencer patterned somewhat after the modular analog step sequencers.
